继发性眼眶黑色素瘤的研究进展

摘要: 继发性眼眶黑色素瘤在眼眶黑色素瘤中,属较为常见的一类,其来源复杂,最常见原发肿瘤为脉络膜黑色素瘤,其次为结膜及皮肤黑色素瘤,少数来源于眼睑、鼻窦黑色素瘤。本病的原发疾病不同,其临床表现也不尽相同,常见症状有眼球突出、复视、视力下降或丧失、疼痛等。诊断通常较困难,需要与原发性眼眶黑色素瘤相鉴别。病理组织学检查可以确定黑色素瘤的诊断,CT、MRI可以辅助诊断并帮助寻找原发病灶。手术切除是本病的主要治疗手段,辅助治疗包括放疗、化疗、免疫治疗等。目前,免疫治疗在处置远处转移来源的继发性眼眶黑色素瘤方面显示出良好的发展前景。由于原发肿瘤不同,本病预后较难预估。(国际眼科纵览,2022, 46:91-96)

Abstract: Secondary orbital melanoma is a common type of orbital melanoma and has complicated origins. The most common primary tumor is choroidal melanoma, followed by conjunctival melanoma and cutaneous melanoma, and rarely caused by eyelid and paranasal sinus melanoma. According to the difference of primary tumor, the symptoms and signs manifest various, such as proptosis, diplopia, visual damage and pain. Diagnosis of this disease tend to be difficult. It is spiny and vital to be distinguish from primary orbital melanoma. Histopathological examination can help clinicians gain the diagnosis of melanoma, CT and MRI show the primary focus. Surgery is the main treatment, and adjuvant radiotherapy and chemotherapy should be considered. At present, immunotherapy has shown promising prospects in the theatment of secondary orbital melanoma caused by distant metastasis. As a kind of highly malignant tumor, prognosis of this disease is difficult to predict because of its perplexing origins as early as possible. (Int Rev Ophthalmol, 2022, 46: 91-96)
